This project consists of an approximately 50,000-square-foot, 60 unit transitional care and skilled nursing facility for Thrive.

This project consists of an approximately 50,000 SF transitional care and skilled nursing facility developed on a 4.25-acre site, with additional improvements completed on an adjacent 2.25-acre property. The facility includes 60 resident units with 68 beds and is licensed as a skilled nursing facility by the Illinois Department of Public Health (IDPH), supporting both short-term rehabilitation and long-term skilled nursing care.

The building is constructed using wood framing with structural steel elements, including a clerestory lobby that introduces natural light into the main common area. Resident amenities include restaurant-style dining, a community room, physical therapy space, and a spa, creating a comprehensive environment that supports rehabilitation, wellness, and daily living.

The project was delivered as a HUD-financed healthcare development, requiring coordination with applicable regulatory and financing requirements. The facility was engineered to provide a durable, resident-focused healthcare environment with integrated therapy, hospitality, and support spaces.
