This project consists of an approximately 95,000 SF single-story transitional care and skilled nursing facility designed to support short-term rehabilitation and post-acute care services. The facility includes 127 resident units with a total of 185 beds, comprised of 69 private units and 58 semi-private units, providing accommodations for patients requiring varying levels of skilled nursing and transitional care.

The building is designed as a full I-2 healthcare occupancy and meets applicable Illinois Department of Public Health (IDPH) requirements for skilled nursing facilities. The single-story configuration supports efficient patient circulation, accessibility, and staff operations while providing dedicated space for nursing care and rehabilitation services.

Operational support areas include a full-service commercial kitchen and onsite laundry facilities, providing essential services for residents and staff. The project was delivered as a HUD Section 232-financed development, requiring coordination with applicable healthcare construction and financing requirements. The facility was engineered to provide a durable, code-compliant healthcare environment with efficient building systems and infrastructure designed to support long-term skilled nursing operations.
